The Vestec Municipal Authority as the procedural guardian informs that on 09.09.2022 it received a payment order for Mr. Pham Vu Hoang in favour of the General Health Insurance Company of the Czech Republic. The payment order is available for inspection by Mr. Pham Vu Hoang during office hours at the office of the Vestec Municipal Office, Vestecká 3.

The time limit for payment is 15 days from the delivery of this assessment, or 15 days from the entry into force of the assessment to pay the penalty.
